Facility:
We are proud to provide a state-of-the-art facility for the highest quality dental care available. It is one of our top priorities to protect the well-being of our valued patients. For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.
Appointments:
Our office attempts to schedule appointment at your convenience and when the time is available. Since in our office that time is reserved exclusively for each patient, we ask that you please notify our office in 48 hours in advance of your appointment if you are not able to keep your appointment. We realize that unexpected things can happen, but we ask for your assistance in this regard. A fee may be charged if a patient does not show up for an appointment without sufficient notice. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.
Initial Appointment:
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
1. Your referral slip and any X-rays if applicable. 2. A list of medications you are presently taking. 3. If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claim. All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Staff: Our experienced staffs able to meet every patient's needs and will be more than happy to assist you with your dental and personal needs.
Financial / Patient Billing:
For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 703-764-0650. Many times, a simple telephone call will clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We accept a wide range of insurances. Please call the office to see if we can accommodate your plan. If we have received all of your insurance information on the day of your appointment, we will be happy to file your claim for you. We will make you familiar with your insurance benefits. At Fair Lakes Dental and Dr. Dehyar & Associates we work closely with your insurance company for your maximum benefits. We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.
